A 53-year-old man will likely be extradited to New South Wales today, where he will be charged with murder.
He was arrested by Queensland police at a Bonogin home on Monday evening, following an extensive investigation into a fatal crash on the Tweed.
The man's car crashed into the Tweed River at Tweed Valley Way in Tumbulgum on October 18, 2015.
He managed to escape through the driver's side window, before apparently going back to rescue his wife from the passenger seat.
The driver then apparently gave the 57-year-old CPR until paramedics arrived at the scene, but she died in hospital two days later.
Police expected to apply for the man's extradition in the Southport Magistrates Court on Tuesday.
