Paramedics say it's an absolute miracle a 14-year-old boy managed to survive after spending two hours treading water without a life jacket off the coast of Gladstone.

The teen fell out of his grandparents' yacht on Sunday, they didn't notice he was missing until they arrived back at the marina, then making a panicked call to authorities.

Before emergency crews could get to him though, another boat spotted the boy drifting in choppy waters and pulled him to safety.

QLD Ambulance spokesman Chris Pereria said the average person could tread water without much difficulty for about 30 minutes.

"He was very, very lucky to survive, he did this remarkable act without a life jacket as well," he said.

The teen did swallow quite a lot of water, but it now stable in hospital.
