A paramedic who was the first emergency worker on-site at a workplace accident in western Sydney on Wednesday has described the harrowing moment he realised the injured man had already died.

Tim Macorella, who works with the Special Operations Unit, had been sent to tend to 38-year-old Mohammed Riche, who was reported to have fallen five stories down a shaft at the Geneva Jordan Springs work site.

But Macorella said it was actually much further than that.
