A coroner has recommended sweeping changes be made to both pre and post-operational procedures at the Toowoomba Hospital after the inquest into the death of Oakey woman Gwendoline Mead in 2015.

Mrs Mead was at the hospital for elective surgery to treat bowel cancer.

Coroner Ainslie Kirkegaard identified a number of missed opportunities by the hospital which could have optimised her care.

Mrs Mead passed away twelve days after surgery.

The hospital has since implanted the changes
